Authorization for Third-Party Representation
The NPDF now requires a notarized Authorization for Third-Party Representation whenever a member requests that another person handle their NPDF-related business. This form ensures that the member’s wishes are clearly documented and that the NPDF can safely communicate with the named representative.
Why This Is Important
In the past, individuals have misrepresented members, charged excessive fees, or acted without proper authorization. To protect our members and the Foundation, we can only discuss a member’s case with an authorized representative when we have a signed and notarized form on file.
How It Works
- The member names their authorized representative in the form.
- The member signs and has the form notarized.
- The form includes a rescind clause allowing the member to revoke authorization at any time, and a hold harmless clause protecting the Foundation from liability.
- Once submitted and approved, NPDF staff can speak directly with the authorized representative.
